Studying the impact of cancer on large populations can provide important information that influences practices, policies, and programs that directly affect the health of millions of people each year. To improve cancer outcomes, it is essential to identify and test methods for more effectively disseminating information about new approaches for cancer prevention, risk assessment, screening, prognosis, treatment, and survivorship. Interventions must fit within real-world public health and clinical settings and be accessible and understandable to practitioners and the public.

Antibody towards S-Hepcidin 25 helps anemic cancerpatients

S-Hepcidin 25 increases in inflammation due to IL-6. If above reference interval, I don´t think there is any use to give iron to a cancerpatient with functional irondeficiency since Hepcidin will block the release of iron from gut to the bloodstream.

What is the research problem :

An antibody towards Hepcidin would be needed to cure anemia in cancerpatients with inflammation and high hepcidin. I don´t

think that this type of antibody exists.

How will your solution make a difference :

Patients with functional irondeficiency and anemia could absorb iron again, the anemia would go away and they would feel better.

What is your proposed solution :

If an antibody toward Hepcidin could be made patients with functional anemi could be helped, iron leave duodenum and enter the blood and bonemarrow and erytropoesis increase. Blocking/reduction of IL-6 would have the same effect.

Use animal spontaneous occurred cancer model in human research

The spontaneous occurred cancer in animal is a precious resource to do the cancer research for human, such as dogs.

What is the research problem :

The current animal model for cancer research relies on nonspontaneous animal model but this model fail to mimic the human patients.

How will your solution make a difference :

The animal spontaneous occurred cancer model could serve as a bridge to fill the gap between animal and human patients and this research strategy could benefit both human and animal cancer patients.

What is your proposed solution :

Using the spontaneous occurred animal cancer for research which will be a better way to mimic the human patients since human and animals share the similar signaling pathways in certain cancers.

CryoPen: Gasless Cryotherapy

Cryotherapy requires cryogen gas - either CO2 or N20 - which is often difficult to procure, cumbersome to transport and expensive. Finding an alternative that is effective could allow more women to obtain life-saving treatments.

What is the research problem :

Cryotherapy has been the mainstay of treatment for cervical pre-cancer in LMICs but it is expensive, difficult to procure and cumbersome to transport. Many women do not get the treatments they need because of shortages of cryogen gas.

How will your solution make a difference :

Eliminating the need for cryogen gas for cryotherapy can reduce costs, remove cryogen gas procurement issues and maintain high treatment efficacy and provider and patient acceptability. More women could then obtain life-saving treatments.

What is your proposed solution :

CryoPen, a gasless cryotherapy, can be used in lieu of a traditional gas-based system. This will reduce both costs and the many logistical problems of procuring cryogen gas. The CryoPen uses sterling cooler technology to chill a copper core by plugging the unit into a regular electric outlet or by using a car battery. The only consumable is very inexpensive ethanol. It is easy to use and has been well accepted by both patients and providers.

HPV Self-Sampling: Increasing Populatoin Coverage

Traditional screening for cervical cancer requires a provider to do a speculum exam. Allowing women to obtain their own samples can obviate the need for an exam and can make cervical cancer screening accessible to many more women.

What is the research problem :

Women in LMICs do not get screened for cervical pre-cancer for many reasons, one of which is the need to make a special trip to a provider for an exam, which can be logistically difficult and scary.

How will your solution make a difference :

More women will be able to be screened for cervical cancer, enabling providers to detect pre-cancerous lesions early enough to provide treatments that will prevent invasive disease and save lives.

What is your proposed solution :

Instituting a program of self-collection for high risk HPV in 2 LMICs: El Salvador and Haiti to prove that it is acceptable to women and increases screening coverage. Community Health promoters will be trained in how to instruct women to perform self collection and then find women who have not been screened for more than 3 years. Women who are HPV+ will then be treated.

Develop an independant platform to incorporate IT proposals

Highly integrated Health Information Systems are complex and expensive to modify. A standalone system that takes an HL7 feed could be deployed with less impact on existing health systems, An incentive from CMS would encourage acceptance.

What is the research problem :

The existing Health Information Systems in hospitals are complex and often very expensive to modify. This creates logistical barriers for implementation.

How will your solution make a difference :

Diagnostic and clinical data are important to build the complete picture of the disease. Machine learning has been mentioned and big data solutions allow for analysis of massive amounts of data. Deploy the solutions as fast as possible.

What is your proposed solution :

A standalone Data Collection Engine for Cancer Research can be purpose built to incorporate the many current and future needs of the research and medical communities. A compartmentalized application that is designed to interface to the related applications could be developed faster with emphasis on control and security. An example that comes to mind is connectopensource.org.

Individual reference intervals and screening of healthy adults

Take screening tests of common clinical chemistry analytes on healthy individuals. Compare lab.results from when the person feels ill with results taken when the person was feeling healthy to be able to notice changes in the results.

What is the research problem :

Reference intervals in clinical chemistry shows results of 95% of the population. When comparing an individual´s result with the reference interval you might not notice changes or disease. Cancer might not be noticed because of this.

How will your solution make a difference :

Some cancer´s could be found earlier and the chance of curing the patient increases. this is costeffective. I have many ideas about functions in this system and I could help you with designing/creating it, please contact me for more information.

What is your proposed solution :

It is much better to compare an individuals result to that persons old results to see if there has been a change. It increases the chance of finding cancer early and increases the chance of curing the person from cancer. I can help you to develop/design a system like this whose aim is individual reference intervals that will help the physician to diagnose a cancer early.

Thus, based on the findings of this literature research, the relevance and validity of assessment cannot to be determined or established in measuring personal characteristics and their relationship to service excellence.

The healthcare workforce injury rate is 30 times higher than other industries. The implementation science framework can be used to  identify the barriers to their successful implementation and strategies for overcoming them.

What is your proposed solution :

As learners and health leaders, in order to transform primary care in the positive direction, the United States must instill leadership, teamwork, and willingness to change within these physicians. Different decision makers and stakeholders are likely to be involved in 3 different types of innovations: biomedical innovations, changes in care delivery, and new ways of engaging patients in their own care.

Routing the right information to the right providers

Determining the provider in patients who have multiple care teams is not as easy as it sounds.Given that health care is reliant on shifts and changes of teams,routing new information or changes in information is crucial to ensure seamless continuity.

The programming in current EMRs is not able to do this well without manual flagging and forwarding mechanisms.This is prone to human error because of human recall.EMR vendors are not incentivized for solutions and they lack needed big data expertise

Tackling this issue ensures continuity of care without misrouting errors and information lapses.Having test data available democratizes the opportunity for experts to take this on.Envision the right information going to the right provider seamlessly.

What is your proposed solution :

Machine learning can use probabilistic and similarity models on EMR data to focus the likelihood of who currently is the provider.Envision if someone orders a drug or lab test,looks at a pathology report;such data elements factor into a machine learning model.Many institutions have data repositories setup differently and "silo'd".Multiple iterations of representative test data should be available to data science teams for model simulation.

Disseminate Evidence- Based Psychosocial Treatments in Community

Many communities lack evidence based treatments that enhance health and may extend life

What is the research problem :

Patients in many areas cannot access the type of evidence-based treatment to reduce anxiety and stress and cope with treatment side effects, yielding a significant gap in cancer care.

How will your solution make a difference :

RCTs have tested psychosocial treatments and found them efficacious in reducing stress and improving qual of life & health, with selected demonstrations of enhanced immunity & reduced risk of recurrence or cancer death. More people could benefit.

What is your proposed solution :

Provider education and training is needed to move effective interventions from research settings to community. Cancer diagnosis and initiation of treatment are enormous stressors for patients and if psychosocial needs are not addressed early, patients may develop stress, anxiety, depression or increased physical symptoms complicating treatment.

Cancer patients need to be cared based on their cultural context. The knowledge available for some countries is based on evidence of Western countries.

How will your solution make a difference :

Including the evidence made by other researchers rather Western ones can help to improve knowledge in cancer care which is more comprehent and worldwide. This can help to solve more challenges in dimensions like spiritual care.

What is your proposed solution :

Researchers in every society should plan priorities of the research in these areas and the evidence based on these studies should be reported in a way that they can be included in the knowledge presented in textbooks of cancer care. Global conferences should be planned in areas that health care providers in areas like Middle East could participate and report their findings.

Revise procedures to capture ALL metastatic cancer diagnosis

Current EHR systems and SEER data collection do not capture when a patient's cancer recurs or progresses to become metastatic. Thus statistics do not accurately capture risk of metastatic disease.

What is the research problem :

SEER database and most EHRs do not capture when a patient's cancer has progressed to a more advanced stage or become metastatic. Thus SEER statistics do not reflect true incidence or prevalence of metastatic cancers.

How will your solution make a difference :

Metastatic cancer prevalence and incidence data will be more accurate. Funding for metastases research will increase. Survivorship plans will reflect correct diagnosis. Understanding and big data analysis of cancer progression will improve.

What is your proposed solution :

Require EHRs, SEER, and other databases to have fields that capture when a metastatic diagnosis occurs, whether at initial diagnosis, as a progression, or as a recurrence.

What is the research problem :

Substantial info about cancer/research is available online from knowledgeable sources such as NCI, ASCO, AACR, advocacy groups, and select patient communities. Unfortunately, many healthcare providers believe Internet contains too much "bad" info.

How will your solution make a difference :

Patients become more engaged when they have access to knowledgeable and responsive info resources. Engaged patients have better outcome and lower healthcare costs. Patients who can find answers 24/7 reduce workload for healthcare providers.

What is your proposed solution :

Provide incentives for healthcare providers and facilities to identify and recommend authoritative online information resources to their patients (e.g., make it a way to fulfill meaningful use requirements?)
